Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform comprehensive in-home health assessments for Medicare Advantage and other plan members in a field-based setting. You will educate patients, close care gaps, and collaborate with primary care providers to improve outcomes.

The role offers flexible scheduling (20–40 hours/week), no on-call, no weekends, and a sign-on bonus with competitive total compensation.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the state you will be practicing in
• Active and unrestricted driver’s license, access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, medications, vitals and exam
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
