Job summary

UnitedHealth Group's Optum HouseCalls team is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members in a flexible, field-based role. The position emphasizes quality outcomes, patient education, and coordination with PCPs to close gaps in care.

The role offers flexible scheduling between 8am-8pm over a 7-day week, no on-call, no weekends, and a $10,000 sign-on bonus for external candidates.

Qualifications

• Active clinical experience in family, geriatric, or home health nursing/PA/NP roles.
• Strong communication skills with geriatric/Medicare populations.
• Ability to transport equipment up to 30 pounds and navigate stairs for home visits.
• Licensure in the practicing state without disciplinary actions; valid driver’s license.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning; coordinate with PCPs to close care gaps.
• Educate members on disease processes, medications, and adherence; share care plan details with PCPs.
• Identify urgent situations and intervene, ensuring safe home environments during visits.
• Address social determinants of health and provide appropriate referrals.
• Collaborate with primary care providers and other care team members to improve outcomes.
