Job summary

UnitedHealth Group, via Optum Home & Community Care, seeks a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to conduct in-home health assessments for Medicare Advantage and other plan members, focusing on education, gap closure, and care coordination.

This field-based role offers flexible scheduling (20–40 hours/week), no on-call, no weekends or holidays, a $10,000 sign-on bonus, and a path to growth within an interdisciplinary care team.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Current, unrestricted license in the practicing state with no disciplinary actions.
• Valid driver's license and access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, medication reconciliation, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
