Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

You will work in a flexible schedule with no on-call, no weekends, and no holidays, supported by a multidisciplinary team to improve patient outcomes and deliver care in patients' homes.

Formación

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted Texas license or ability to obtain by start date.
• Access to reliable transportation for home visits.

Responsabilidades

• Conduct comprehensive in-home assessments including vitals, history, and exam.
• Document diagnoses for care management and planning.
• Communicate findings to PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate patients on diseases, meds, and adherence.
• Address social determinants of health and provide referrals.
