Job summary

Qualifications

• Must be able to read, write, speak, and understand English.
• Ability to perform physically demanding tasks (lifting up to 50 pounds).
• Must be able to travel as needed and maintain patient safety.

Responsibilities

• Implement the Plan of Care developed by the Physical Therapist.
• Communicate with PT, physician and staff about patient care.
• Collaborate on patient assessments (OASIS).
• Educate patients and families on exercise programs and safety.
• Participate in interdisciplinary team meetings and discharge planning.
• Perform home visits to implement the Care Plan at patients' homes.
• Maintain clean, safe treatment areas and equipment.
• Deliver PT treatments per productivity standards.
