Qualifications

• Experience: one year preferred; new graduates welcome.
• Must read, write, speak, and understand English.

Responsibilities

• Implements the plan of care developed by the Physical Therapist under supervision.
• Communicates with the Physical Therapist and staff about patient care needs.
• Participates in patient assessments and interdisciplinary team meetings.
• Conducts home visits to guide therapy and self-care activities for safety and function.
• Maintains patient records and ensures accuracy in electronic documentation.
