Job summary

Boost Home Healthcare is seeking a licensed Physical Therapist Assistant to provide physician-ordered PT in patients' homes across Lorain County. The role emphasizes improving functional mobility, documentation, and coordination with the care team.

The ideal candidate has PTA degree, Ohio license, at least 6 months of PTA experience (home health preferred), and valid driver's license with reliable transport. Flexible work-from-home options available.

Qualifications

• Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
• Currently licensed in the state in which practicing.
• Minimum of six (6) months experience as a Physical Therapy Assistant.
• Two (2) years’ experience preferred in a home health setting
• Knowledgeable and experience with Medicare OASIS documentation
• Licensed driver with automobile insured in accordance with state/Agency requirements.

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Documents all services provided, observations, client progress, and recommendations.
• Participated in interdisciplinary activities per Agency guidelines.
