Job summary

Ohio Living is seeking a Licensed Physical Therapy Assistant (LPTA) to provide therapy services under the supervision of a Physical Therapist. The role focuses on implementing care plans, collaborative assessments, and patient education in a home health or facility setting.

Applicants must hold an Ohio LPTA license, have an associate degree, and be comfortable serving geriatric patients. New graduates are welcome; driving and visiting patients at home may be required.

Qualifications

• Associate degree in Physical Therapy Assistant Technology from a CAPTE-accredited program or equivalent.
• Current unencumbered LPTA license for Ohio.
• Experience in geriatric and physical therapy care preferred; new graduates welcome.

Responsibilities

• Implements the Plan of Care developed by the Physical Therapist according to physician orders.
• Collaborates with the clinical team to execute patient assessments (OASIS).
• Educates patients and families on exercise programs and self-care activities.
• Maintains patient records and ensures accurate documentation in EMR.
• Performs home visits to implement the Plan of Care and promote independence.
• Ensures clean and safe treatment areas and equipment, and adheres to productivity standards.
