Job summary

ProHealth Home Health & Hospice is seeking a Licensed Physical Therapy Assistant (LPTA) to administer physical therapy to patients in their homes under physician orders and a PT plan of care.

The role requires an Associate degree in Physical Therapy Assistance, AL licensure, and a preference for at least two years of prior experience. The LPTA will collaborate with the PT and care team to support patient recovery and independence.

Qualifications

• Graduate from a two-year college level program approved by the American Physical Therapy Association.
• Currently licensed, registered or certified in the state in which practicing.
• Two years of experience preferred.
• Acceptance of philosophy and goals of Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides physician prescribed physical therapy under a plan of care established by the PT.
• Implements treatment plans to improve or minimize residual disabilities.
• Supports patient independence and returns individuals to optimal functioning.
• Collaborates with the PT and other health care personnel on patient care planning.
• Documents care provided and progress notes in a timely manner as per Agency policy.
