Ashealthnet is seeking a Physical Therapist Assistant I (PTA) in Cleveland, OH to provide skilled PTA services in patients’ homes under a Physical Therapist’s direction. You will educate patients and families, document care, and ensure safety and compliance with regulations.

The role requires at least one year of PTA experience in an acute setting, a valid Ohio PTA license, and strong communication with patients and healthcare teams. Home health experience is preferred and driving is required.

Formación

• Minimum of one year of PTA experience in an acute setting.
• Home care experience is desired.
• Strong communication with patients, families, and physicians.
• Knowledgeable about therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented and reliable.
• Valid driver’s license and auto insurance in your name.

Responsabilidades

• Provide skilled PTA services in the patient’s home as prescribed by the Physical Therapist.
• Observe, record, and report patient progress and reactions.
• Educate patients and families on therapy techniques and safety measures.
• Complete clinical documentation and charting within agency guidelines.
• Coordinate with the Physical Therapist and care team to ensure continuity of care.
