Job summary

UnitedHealth Group is seeking a Home Health Licensed Practical Nurse to provide direct nursing care in patients’ homes, under supervision, and coordinate other patient care activities. You will document visits and participate in care coordination to ensure high-quality outcomes.

Required qualifications include an LPN license, CPR certification (or within 90 days), a valid driver's license, and the ability to work in diverse home environments.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws, and in coordination with the health care team, following infection control procedures.
• Documents patient visits thoroughly in the electronic medical record and adheres to regulatory and payer requirements.
• Participates in the agency's care coordination process and communicates patient needs and changes in condition to the team and caregivers.
