Job summary

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• CPR certification preferred or ability to obtain within 90 days
• Ability to work flexible hours to meet client needs
• Ability to manage multiple tasks and work independently

Responsibilities

• Provides high quality clinical services within the scope of practice and coordinates with health-care team
• Documents patient visits thoroughly in the electronic medical record
• Participates in care coordination and reports changes in patient condition
• Ability to work in diverse home environments and with various patient populations
