Job summary

UnitedHealth Group seeks a Licensed Practical Nurse (LPN) for Home Health to deliver high-quality patient care in patients' homes. You will follow the plan of care, coordinate with the health care team, and maintain infection control standards.

Responsibilities include accurate documentation in the EMR, active participation in care coordination, and effective communication with patients and families. A valid LPN license is required, with opportunities to grow within a nationwide organization.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks and work independently.
• Good communication, writing, and organizational skills.
• Flexible hours as required to meet client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws, following the plan of care and infection control procedures.
• Documents patient visits thoroughly and completely in the electronic medical record.
• Participates in care coordination, reports patient needs, attends case conferences, and communicates with patient, family, physician and team.
• Ability to function in any home situation regardless of client demographics.
