Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide direct home health care under physician guidance, documenting visits in the EMR and coordinating patient care within a dedicated team. You will operate under supervision to ensure high-quality patient outcomes.

The role requires a current LPN licensure, valid driving license, and ability to work flexible hours. CPR certification is preferred, with a strong emphasis on independent work and effective communication with patients

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.
• Ability to work flexible hours to meet client needs.
• Ability to multitask and work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care.
• Documents patient visits thoroughly and completely in the electronic medical record.
• Participates in care coordination, reporting changes in condition and communicating with patient, family, physician and care team.
• Adaptability to work in varied home situations regardless of client demographics.
