Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide direct home-based nursing care to acutely and chronically ill patients within the assigned service area. You will document visits, coordinate with the care team, and ensure quality patient outcomes under supervision.

Requirements include current LPN licensure, valid driver's license, and reliable transportation. CPR certification is preferred; flexibility and strong organizational skills are valued to manage multiple tasks and

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire
• Ability to work flexible hours as required to meet identified client needs
• Ability to manage multiple tasks simultaneously
• Able to work independently
• Good communication, writing, and organizational skills

Responsibilities

• Provides high quality clinical nursing services within the scope defined by state laws and plan of care.
• Documents patient visits thoroughly in the EMR and ensures compliance with regulatory requirements.
• Participates in care coordination, communicating patient needs and changes in condition to the team.
• Functions effectively in diverse home settings and with varied patient needs.
