Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by the state laws governing nursing.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reports patient needs, and communicates with family, physician, and care team.
• Function in diverse home situations and uphold inclusive care for all patients.
