Qualifications

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician and case managers.
• Function in diverse home situations while upholding patient rights and dignity.
