Job summary

Optum is seeking a Licensed Practical Nurse to provide direct home health care under physician orders, documenting progress notes and coordinating patient services in the assigned geographic area.

The role requires an active LPN license, a valid driver’s license, and the ability to work in patients’ homes with diverse needs. CPR certification is preferred, and flexible hours may be required to meet client needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care.
• Documents patient visits thoroughly and completely in the electronic medical record.
• Participates in care coordination including timely reporting of patient needs and condition changes.
• Ability to function in any home situation regardless of patient demographics.
