AHN Healthcare@Home is hiring a Licensed Practical Nurse to provide skilled nursing care in patients' homes in the Clinton, Oakdale, and Coraopolis areas. This role offers a $10,000 first-year success bonus for achieving key milestones.

You’ll work autonomously within an interdisciplinary team, travel to patient homes, and participate in a comprehensive orientation and ongoing growth opportunities. We value autonomy, meaningful patient relationships, and a clear path for advancement.

Qualificações

• Active LPN license in the state you work.
• Minimum two years of LPN experience.
• Ability to drive to patient locations ~20% of your day.
• Excellence in EMR documentation and charting.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you


• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.


• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines


• Maintain accurate, timely, and compliant EMR documentation

• Work independently, but never alone


• Travel to patient homes within assigned territory

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
