Job summary

AHN Healthcare@Home is seeking a Home Health LPN in Bridgeville, PA to provide skilled nursing care in patients' homes within the McDonald, Bridgeville, and Burgettstown areas. A $10,000 first-year success bonus is offered for achieving key milestones.

You will travel to patient homes, work with an interdisciplinary team, and document care in the EMR system. The role includes autonomy, opportunities for growth, and a comprehensive benefits package.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work.
• Two years of LPN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines.
• Maintain accurate, timely, and compliant EMR documentation.
• Travel to patient homes within assigned territory.
