Job summary

AHN Healthcare@Home is seeking a Licensed Practical Nurse to provide skilled nursing care in patients’ homes across McDonald, Bridgeville, and Burgettstown areas. This role includes a $10,000 first year success bonus and independent fieldwork with support from a dedicated education team.

You will perform patient assessments, medication management, wound care, and patient education while maintaining accurate EMR documentation. A valid LPN license and driving ability are required.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience.
• Current driver’s license with ability to travel ~20% of the day

Responsibilities

• Deliver skilled, patient-centered care in home health settings with interdisciplinary support.
• Perform comprehensive patient assessments including vitals, meds, wound care, and health status.
• Educate patients and caregivers on disease management, meds, and prescribed treatments and ensure adherence.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
• Maintain accurate, timely, and compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
