Residential Home Health and Hospice in Pennsylvania is hiring a Licensed Practical Nurse to provide skilled nursing care to patients in their homes in the Duncannon area. This role includes a $10,000 first-year success bonus for achieving key milestones.

You will deliver patient-centered care, conduct assessments, manage medications, educate families, maintain EMR documentation, and travel to patient homes within your territory.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience in home health or clinical care.
• Valid driver’s license and ability to spend ~20% of your workday driving.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines.
• Maintain accurate, timely, and compliant EMR documentation.
• Travel to patient homes within assigned territory.

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
