Job summary

AHN Healthcare@Home is seeking a Licensed Practical Nurse to provide skilled nursing care in patients' homes in the Apollo, PA area. A $10,000 first year success bonus is offered for achieving key milestones.

You will deliver patient-centered care in a home health setting, collaborate with an interdisciplinary team, and educate patients and caregivers on disease management and prescribed treatments. You will travel to patient homes within your territory and document in the EMR system.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work.
• Two years of LPN experience.
• Valid driver’s license with ability to drive ~20% of day to patient homes

Responsibilities
