Qualifications

• The Intake Coordinator must have healthcare experience, preferably in referrals/intake in a home health or hospice environment.
• A nursing background is preferred but not required.
• Demonstrates good communication, negotiation, and public relations skills.
• Demonstrates autonomy, assertiveness, and cooperation in performing job responsibilities.

Responsibilities

• Directs all daily patient referral and intake operations including overseeing intake policies.
• Ensures compliance with state, federal, and Joint Commission requirements.
• Assists with implementing improved work methods to admit patients per policy.
• Maintains positive relationships with referral sources.
• Supports patient education and transition to home care; coordinates plan of care.
• Oversees insurance verification and authorization processes for maximum reimbursement.
• Shaping community perception of Sacred Heart Home Health as a high-quality provider.
• Gathers and reports referral statistics and trends.
• Keeps knowledge of home health contracts and ensures admissions align with provisions.
• Assists referral sources in accessing community resources when services are not provided.
