Choice Health at Home is seeking an organized, customer-focused Intake Referral Coordinator to manage the initial patient referral process. You will serve as a key liaison between hospitals, physicians, patients, families, and clinical staff to support a seamless start of care.
Professional experience working with insurance or in a home health office is required.
Receive, review, and accurately document new referrals in a timely manner.
Verify insurance eligibility, benefits, and payer sources.
Obtain, maintain, and track insurance authorizations and initial certifications.
Accurately enter patient and referral information into the electronic medical record.
Coordinate with Nursing Supervisors, patients, and the interdisciplinary team to support timely care.
Communicate effectively with patients, families, staff, healthcare professionals, and referral sources.
Respond to agency and patient needs professionally, using sound judgment and problem-solving skills.
Participate in agency training and the Quality Assessment and Performance Improvement (QAPI) program.
Follow agency policies and support a commitment to quality patient care.
Required:
Professional experience working with insurance or in a home health office.
Excellent computer, organizational, and customer service skills.
Strong communication skills and attention to detail.
Valid driver’s license and auto insurance.
Preferred:
At least one year of referral intake experience.
Experience with third-party payers, Medicare, and Medicaid.
Medical field background and familiarity with medical terminology.
