Job summary

Puget Sound Home Health & Hospice in Tacoma, WA is seeking a full-time Home Health & Hospice Patient Scheduling Coordinator to join a dedicated team supporting clinical staff and patients. This on-site role requires at least 2 years of medical office scheduling experience and familiarity with Home Care Home Base.

Qualifications

• At least 2 years of medical office experience with scheduling duties.
• Experience with Home Care Home Base is strongly preferred.
• This is an on-site role in Tacoma, WA.

Responsibilities

• Act as primary home health and hospice scheduler for clinical staff.
• Ensures optimum staffing solutions by maintaining staff demographics, skill sets, availability, and productivity levels.
• Interface with contract, clinical, and office staff with a focus on collaboration and positive working relationships.
• Serves as the primary responder to all inbound calls and inquiries.
• Provides excellent customer service to patients, referral sources, and community contacts.
• Accurately addresses requests for schedule changes, caseload reassignments, recertification of services, new referral scheduling, and all other visit workflow as assigned.
• Educates and enforces deadlines, schedule updates, and workflow requirements.
