Qualifications

Demonstrated excellent presentation, negotiation and relationship-building skills.
Proficiency with Microsoft Outlook and CRM software requirements.

Responsibilities

Build and maintain relationships with target referral sources to expand care access.
Implement, manage, and document sales activities with multiple contacts in each referral source.
Understand customer needs to provide customized solutions and earn referrals.
Educate community on home health/hospice services and solutions.
Serve as liaison between community, patients, families, and agencies.
