Better Health Group is seeking a Health Coach who will engage patients primarily by telephone to facilitate transitions of care after hospitalizations and other events. You will coordinate preventive care, support medication adherence, and identify barriers to accessing care while documenting activities in our systems.

The role emphasizes proactive outreach, collaboration with providers and care teams, and a commitment to reducing readmissions and improving overall patient well-being.

Qualifications

• High school diploma or equivalent required; associate's or bachelor's degree in healthcare, health promotion, public health, psychology, or related field preferred.
• 1+ year of experience in healthcare, patient outreach, care coordination, health coaching, customer service, or related field preferred.
• Familiarity with Transitional Care Management (TCM) and Social Determinants of Health (SDOH) preferred.
• Strong interpersonal and patient engagement skills; ability to establish rapport, empathy, and effective communication with diverse patient populations.
• Excellent verbal and written communication; experience in telephonic outreach and coordination with patients and providers.
• Strong organizational and time-management skills; ability to manage multiple outreach activities and maintain documentation.
• Strong critical thinking and problem-solving; identify barriers to care and escalation per protocols.
• Ability to work independently while collaborating with clinical and operational teams.
• Proficiency with Google Workspace and navigating EMRs and patient management systems.
• Commitment to confidentiality and HIPAA compliance.

Responsabilités

• Conduct proactive telephonic outreach to patients to support engagement in recommended healthcare services and preventive care.
• Coordinate Transitional Care Management (TCM) outreach after hospital discharges and other events; schedule timely follow-up appointments.
• Reinforce discharge instructions and identify barriers to follow-up care to reduce avoidable readmissions.
• Schedule Annual Wellness Visits and other preventive care appointments per outreach protocols.
• Conduct Social Determinants of Health (SDOH) surveys and refer needs to appropriate resources.
• Support medication adherence by confirming fills and coordinating with pharmacies or providers.
• Educate patients on healthcare benefits, resources, and services to encourage appropriate utilization.
• Document outreach, appointments, barriers, and follow-up activities in designated systems.
• Collaborate with clinical and operational teams to address patient concerns and ensure continuity of care.
• Maintain confidentiality and adhere to HIPAA and organizational policies.
