Moda Health seeks a dedicated Health Coach to develop and implement behavior-based coaching programs for members, including disease management and health navigation support. You will collaborate with internal teams, external vendors, and employer groups such as the City of Portland to ensure coordinated, continuous care and measurable improvements in outcomes.

You will assess member health, educate on SMART goals, and document progress in accordance with organizational policies.

Qualificações

• Associate degree required or equivalent experience.
• NBHWC training and board certification eligibility preferred.
• Experience coaching health behaviors and managing chronic conditions preferred.

Responsabilidades

• Provide one-on-one coaching through multiple channels (virtual, phone, email).
• Educate and empower members to set SMART health goals and build self-management capacity.
• Conduct initial health assessments and document progress per policies.
• Collaborate with internal/ external teams to achieve better outcomes and lower costs.
• Maintain up-to-date knowledge of vendor health management programs and refer members as appropriate.
