Qualifications

Responsibilities

• Interacts with other facility staff.
• Acts in interdisciplinary team meetings.
• Conducts initial and periodic assessments.
• Administers nursing care to ill, injured, convalescent or disabled participants.
• Advises participants on health maintenance and disease prevention.
• Administers medication if needed.

Compensation

We provide market‑competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full‑time employment is $87,214.40 - $126,900.80 annually. Additional compensation may be available for this role, such as shift differentials, on‑call pay, shift premiums, extra‑shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.

Responsibilities

• The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards.
• Includes interaction with other facility staff.
• Actively participates in interdisciplinary team meetings.
• Conducts initial and periodic assessments.
• Administers nursing care to ill, injured, convalescent or disabled participants.
• Advise participants on health maintenance and disease prevention.
• Administers medication if needed.

Qualifications & Requirements
