Qualifications

• RN license required and maintained per state requirements.
• Minimum two years of nursing experience within the last 36 months.
• Experience in home health and CPR certification required.

Responsibilities

• Coordinate and oversee home health services to meet standards of practice and company procedures.
• Review patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers, and field staff to ensure continuity of care.
• Schedule patients and maintain effective coordination with field clinicians and other service providers.
