Job summary

Enhabit Home Health & Hospice is seeking an experienced registered nurse to lead and coordinate home health services across patient care teams. You will review clinical documentation, support care planning, and serve as the liaison between physicians, patients, and caregivers.

Ideal candidates have an active RN license, CPR certification, 2+ years of nursing experience, and a strong understanding of home health regulations. In-office attendance is required to achieve Enhabit objectives.

Qualifications

• Must be graduate of an approved school of professional nursing.
• CPR certification and valid state license required.
• Minimum of two years nursing experience; home health experience preferred.
• Strong knowledge of federal, state, and local home health regulations.

Responsibilities

• Coordinate and oversee the delivery of home health services per standards and procedures.
• Review and approve start of care, recertification, and evaluation documents.
• Facilitate communication among physicians, patients, caregivers, referral sources and staff.
• Schedule patients to field clinicians and ensure continuity of care.
