Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license in state or compact license.
• Two years nursing experience within the last 36 months.
• One year of home health experience.
• Knowledge of federal, state and local laws for home care operations.

Responsibilities

• Coordinate overall home health services delivery per standards and procedures.
• Review patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referrals, patients, caregivers, and staff.
• Schedule patients with field clinicians and communicate with all stakeholders to maintain care continuity.
