Job summary

Enhabit Home Health & Hospice is seeking a registered nurse to coordinate comprehensive home health services for patients, ensuring care aligns with standards and regulatory requirements.

You will review patient information, facilitate collaboration among physicians, caregivers and staff, and schedule field clinicians to maintain seamless care delivery. Two years of nursing, including home health, is preferred, with CPR and a valid driver license required.

Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements or compact license rules.
• Two years nursing experience within the last 36 months.
• One year of experience in home health.
• Knowledge of federal, state and local home health regulations.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate overall home health services for patients according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients with field clinicians and communicate with all parties to maintain care coordination.
