Job summary

Enhabit, Inc. is seeking a nurse leader to coordinate comprehensive home health services. You will review patient data at start of care and throughout recertification, and liaise with physicians, patients, and caregivers to ensure seamless care delivery.

Required are an RN license, CPR certification, and at least two years of nursing experience with one year in home health. A valid driver license and auto insurance are also required.

Qualifications

• Graduate of an approved school of professional nursing.
• Current valid RN license per state requirements.
• Two years nursing experience within the immediate previous 36 months.
• One year of experience in home health.
• CPR certification is required.
• Valid state driver license and automobile liability insurance.
• Knowledge of federal, state and local laws governing home care operations.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Coordinate home health services to meet standards of practice and company procedures.
• Review patient information at start of care, recertification, or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate to maintain care continuity.
