Job summary

Enhabit Home Health & Hospice is seeking an experienced Registered Nurse to oversee care coordination and field staff scheduling in Texas. You'll ensure care delivery aligns with standards and collaborate with physicians, patients, and caregivers to optimize outcomes.

The role requires RN licensure, CPR, and at least two years of nursing experience, with home health knowledge and strong communication across teams. In-office presence is essential to meet objectives and maintain service excellence.

Qualifications

• Graduate of an approved school of professional nursing.
• CPR certification and valid state license.
• Minimum two years nursing experience; home health experience preferred.
• Strong knowledge of federal, state and local home health regulations.

Responsibilities

• Coordinate delivery of home health services per standards and procedures.
• Review and approve start of care, recertifications, and evaluations.
• Facilitate relationships with physicians, referral sources, patients and caregivers.
• Schedule patients to field clinicians; maintain care coordination and communication.
