Job summary

Qualifications

• RN license or compact state license as required by state regulations.
• CPR certification and ongoing compliance to scope of practice.
• Basic technology skills and ability to operate a mobile device.
• Experience in home health settings is preferred.
• Excellent communication with patients, families and care teams.

Responsibilities

• Coordinate home health services to ensure high-quality patient care.
• Review and approve start of care, recertifications and care evaluations.
• Facilitate communication between physicians, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
