Job summary

Enhabit Home Health & Hospice is seeking a dedicated nursing leader to oversee field-based care coordination and ensure service delivery meets clinical standards. You will partner with physicians, referral sources, patients, and caregivers to optimize care and outcomes.

The role requires RN licensure, 2 years recent nursing experience, home health familiarity, and strong communication skills; hybrid/onsite presence is required to support teamwork and compliance.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• Two years nursing experience within the last 36 months.
• One year home health experience.
• Knowledge of federal, state and local home care laws and regulations.
• CPR certification required.
• Driver license and automobile liability insurance.
• Basic technology skills and familiarity with mobile devices.
• Management experience preferred.
• Experience with Homecare Homebase preferred.
• Bilingual English/Spanish preferred.
• In-office attendance is essential, remote work not offered.

Responsibilities

• Coordinate home health services to meet clinical standards.
• Review patient information at start of care, recertification, resumption, or evaluation.
• Facilitate relationships between physicians, patients, caregivers and staff.
• Ensure scheduling and communication to maintain care coordination and continuity.
