Job summary

Maxim Healthcare Services is seeking a dedicated Home Health Aide (HHA) to provide personal care to patients in their homes according to the plan of care. The role emphasizes compassionate support, safety, and punctual communication with the supervising nurse.

Qualifications

• CPR/First Aid certification required and maintained.
• Ability to read, follow written instructions and document care.
• Effective communication with client, family/caregiver and supervisor.

Responsibilities

• Assist patient with plan of care: baths, oral hygiene, dressing, meals, and daily activities.
• Take and record vitals as ordered (temperature, pulse, respiration, blood pressure).
• Perform safe transfers and ambulation; assist with range of motion and restorative activities as directed.
• Ensure patient safety and proper use of equipment; observe and report changes to nurse.
• Document care and communicate clearly with the client and supervising nurse.

Responsibilities

• Responsible for assisting patient with and per plan of care, the following;
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim background screening examinations
• Participation and completion of Maxim’s competency programMust meet all federal, state, and local requirements
• Must have active hands on CPR/First aide certification
• Must have active negative TB examination
