Concierge Home Care in Trinity, FL is seeking a Home Health Account Executive to grow census in the Trinity territory. You will manage referrals, coordinate services, and advocate for residents as the liaison between clinicians and families.

Ideal candidates bring an active book of business and healthcare sales experience within home health or related fields, with strong communication and driving relationship-building skills.

Qualifications

• Active book of business within the territory required.
• Healthcare sales experience in home health, SNF, DME, hospice, or clinical liaison preferred.
• Clinical background (RN, LPN, MSW, etc.) preferred.

Responsibilities

• Build and maintain strong referral relationships within senior living communities.
• Drive census growth by meeting or exceeding sales goals.
• Identify resident care needs and coordinate appropriate services.
• Act as a resident advocate to ensure smooth transitions to home care.
• Manage the referral-to-admission process efficiently.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Provide education to residents, caregivers, and community partners.

Compensation & Earning Potential

• Base Salary: $75,000 – $90,000+ (based on experience & book of business)

• Strong communication, presentation, and relationship-building skills
• Detail-oriented with strong organizational skills
• Ability to travel within the territory
• Valid driver’s license, auto insurance, and reliable transportation (required)
• Ongoing mentorship and leadership support
