Job summary

Fresenius Medical Care is seeking a Home Therapies RN CAP 2 to coordinate and deliver clinically competent care for an assigned group of patients within the Home Dialysis program. This role may involve delegation to RNs, LVNs/LPNs and Patient Care Technicians under supervision.

The RN CAP 2 contributes to education, quality improvement, and travels between Treasure Coast clinics as needed, maintaining leadership within the care team.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which employed.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Direct and provide safe and effective delivery of dialysis therapies with home dialysis partners.
• Administer medications as prescribed and document effectiveness.
• Train home dialysis patients and partners in safe operation of equipment and supplies.
