Job summary

Fresenius Medical Care is seeking an RN CAP 2 for Home Therapies to coordinate and deliver clinically competent care to an assigned group of home dialysis patients. The role includes assessment, planning, and education, with collaboration across the care team.

Ideal candidates bring an RN license, CPR/BLS, and at least 2 years nephrology experience or CAP 1 progression; CAP 2 provides growth into leadership and quality improvement responsibilities.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state RN licensure.
• Current or successful completion of CPR BLS Certification.
• Minimum 3 months experience home dialysis therapies as an RN.
• Experience in nephrology nursing preferred but not required for entry into CAP 2.

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of Home Therapies patients.
• Perform ongoing data collection and analysis of dialysis data and adjust treatment plans as needed.
• Educate patients and families about treatment plans and home dialysis.
• Lead and mentor new staff and participate in quality improvement activities.
• Ensure safe delivery of dialysis therapies and adherence to policies and procedures.
