Job summary

Fresenius Group is seeking an entry-level Home Therapies RN CAP1 to join our clinic team. You will coordinate patient care for home dialysis patients, perform assessments, plan treatments, and collaborate with physicians to optimize outcomes.

The role emphasizes teaching patients and families, administering care per established protocols, and participating in ongoing quality improvement within the End Stage Kidney Disease program.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure.
• CPR/BLS certification.
• Home dialysis experience (preferred).

Responsibilities

• Provide clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Educate patients and family on home dialysis training and treatment modalities.
• Administer medications and manage dialysis delivery per protocols.
• Coordinate with interdisciplinary team and physician for patient care.
• Document care and outcomes in patient records and participate in quality improvement.
