Job summary

Fresenius Medical Care in the United States is seeking an onsite Home Therapies RN CAP1 to coordinate and deliver home dialysis education and care. You will monitor patients, document progress, and collaborate with physicians to optimize treatment plans.

The role involves training patients and partners, performing nursing assessments, and guiding the care team in a clinic-based setting with potential home visits as needed.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/BLS Certification.
• Entry level for RNs with less than 2 years of Nephrology Nursing experience.
• Minimum of 1 year of RN experience (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Provide clinical care and education for home dialysis patients.
• Administer medications as prescribed.
• Train Home Dialysis patients and partners on equipment and protocols.
• Assess and document patient learning needs; update care plans.
• Collaborate with the interdisciplinary team to ensure quality patient care.
• Lead or assist with staff education and orientation as assigned.
