Qualifications

• Registered Nurse with state licensure and CAP pathway entry requirements.
• Experience in home dialysis education and patient training is preferred.
• Ability to assess, plan, implement and evaluate patient education and care.

Responsibilities

• Coordinate home dialysis training and education for patients and families.
• Assess patient learning needs and provide initial and ongoing education.
• Train Home Dialysis patients and Home Partners on equipment operation and safety.
• Collaborate with the interdisciplinary team to optimize patient care and outcomes.
