Job summary

Fresenius Medical Care in Kentucky is seeking a Home Therapies RN to serve as an onsite patient educator and care coordinator for individuals undergoing home dialysis training and treatment.

You will train patients and Home Partners, perform nursing assessments, document plans of care, administer treatments as prescribed, and collaborate with physicians and staff across shifts, including potential travel to patient homes and on-call coverage.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure required.
• CPR/BLS certification required.
• Minimum 1 year of RN experience; home dialysis preferred.

Responsibilities

• Provide and coordinate clinically competent care per protocols.
• Educate patients about home dialysis modalities and self-care.
• Collaborate with physicians, nurses and caregivers in patient plans.
• Administer medications per approved algorithms and document.
• Support home visits and on-call coverage as required.
