Job summary

Fresenius Medical Care seeks a Home Therapies RN CAP 1 to train and educate dialysis patients in home peritoneal and home hemodialysis, coordinating care in the clinic setting and during initial home visits with an accompanying staff member.

Qualifications

• RN with less than 2 years of nephrology nursing experience.
• Minimum 1 year RN experience (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Provide and coordinate clinically competent care per patient group.
• Assess and document patient learning needs for initial and ongoing education.
• Train patients and Home Partners on safe operation of home dialysis equipment.
• Collaborate with interdisciplinary team to plan and evaluate care.
• Direct and assist with safe delivery of dialysis therapy to assigned patients.
• Participate in on‑call rotations and quality improvement activities.
