Qualifications

• Active RN license in applicable state.
• Experience or training in home dialysis modalities (peritoneal and home hemodialysis).
• Ability to educate patients and families on treatment options and care plans.

Responsibilities

• Coordinate and document care plans with the care team.
• Perform home visits and on-site patient education activities.
