Job summary

CH01 CHE Fresenius Medical Care (Schweiz) AG in Minnesota seeks an Entry-level Home Therapies RN CAP1 to coordinate education and clinical care for home dialysis patients. The role includes on-site supervision, patient assessment, and education, with initial home visits alongside staff.

As part of the End Stage Kidney Disease care team, you will contribute to care planning, protocols, and quality improvement while ensuring safe dialysis delivery and patient empowerment.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Performs essential nursing duties under supervision.
• Collects and analyzes dialysis data and updates the plan of care.
• Educates patients and families on home dialysis training.
• Administers prescribed medications per protocol and documents effectiveness.
• Ensures safe and effective delivery of dialysis therapy and collaborates with the team.
