Job summary

Rehab Without Walls Neuro Rehabilitation in Winston-Salem, NC, seeks an experienced Physical Therapist to deliver functional rehabilitation in home, school, work, and community settings.

You will perform evaluations, develop and adjust treatment plans, re-assess progress, discharge planning, and coordinate with a multidisciplinary team to improve patient outcomes. The role offers autonomy within a collaborative environment and a flexible, hourly schedule.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program
• Current and unrestricted Physical Therapist license in the practicing state
• Current BLS/CPR certification as required by state regulations
• Minimum one year of PT experience preferred
• Knowledge of rehabilitation techniques for neurological injuries
• Strong communication skills, professional interactions
• Ability to lift and move patients per safety procedures

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly reassess effectiveness of treatment plans; attend staffing meetings and family conferences
• Perform discharge evaluations and provide home/community safety recommendations and resources
• Communicate patient needs and progress to the treatment team, physician, client and family
• Collaborate with other disciplines to ensure care coordination and optimal patient outcomes
